Please Note: The practice is now taking taking on new patients.

To register with a GP at this practice, please call in at reception and complete a registration form. You will be registered with one of the practice partners but you can express a preference as to which doctor you see.


If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the 'Send' button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct. You are not registered here until we have your signature on the registration form. Please bring with you photographic proof of identity and a utility bill to confirm your address.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

Your medical records are now able to be transferred electronically to any other GP Practice in England using SystmOne as soon as we are notified of your registration elsewhere. Currently work is being undertaken to ensure that all clinical computer systems in General Practice can work this way. We will be live on 7th November 2014, when we will be able to transfer your medical record electronically on the day we are notified of you registering elsewhere. You don’t have to do anything, it’s just a speedy way of your new GP Surgery having your medical records within a day or two after you have registered with the new practice. Practice Area

Our practice area generally covers areas of Carlton, Gedling, Netherfield, Colwick and Mapperley. We are unable to take on patients who live out side of the area shown below.

If you move outside of this area, you will be asked to change your GP surgery. The Health Authority will be able to provide you with details of local GP’s.

Disabled Access.

Our health centre has suitable access for disabled patients.

All the patient areas including the waiting room, the consulting rooms and the toilets, have wheelchair access. If you require one, there is a wheelchair available at reception

There are designated disabled parking spaces in the car park. There are buttons at key points for you to call for assistance. The main centre reception desk will respond to your call.

A hearing loop is in place at the front reception desk.
